Key Responsibilities

The Social Worker will manage the Social Service Department to ensure residents receive medically related social work services for their physical and mental well-being. Responsibilities include conducting assessments, care planning, discharge planning, and providing interventions for residents, families, and physicians.

Requirements

Candidates must hold a 4-year degree and a valid Social Worker license in the State of Texas. At least 2 years of experience in a skilled nursing facility or hospital setting is preferred.
