Key Responsibilities
Provide comprehensive psychosocial services to terminally ill patients and their families, including assessments and supportive counseling. Manage patient caseloads, coordinate community resources, and participate in interdisciplinary team meetings to ensure quality end-of-life care.
Requirements
Requires a current Social Worker license and a minimum of two years of experience in a healthcare or social services setting. A Bachelor's degree in Social Work is required, with a Master's degree preferred, along with a valid driver's license and auto insurance.
