Key Responsibilities
The Social Worker Assistant will manage resident admission paperwork, care plan development, and resident assessments. They will also oversee the discharge process and facilitate resident care conferences while addressing family concerns.
Requirements
Candidates must be able to work independently while collaborating with the Social Services Director. The role requires strong organizational skills and the ability to handle sensitive resident and family interactions.
