Join HarmonyCares as a Advance Practice Provider – No Weekends, No Holidays in Westland, Michigan, United States. Value-based Advance Practice Provider role managing chronic conditions of high-risk geriatric patients Monday-Friday, no weekends or evenings, in a supportive team setting. This position is w-2 salaried, includes benefits, plus annual quality-based performance.

Key Responsibilities
Provide primary care to high-acuity geriatric patients within their homes. Manage chronic conditions as part of an integrated, team-based, physician-driven model.
Requirements
Must be a licensed Nurse Practitioner or Physician Assistant with a valid DEA license and CPR/BLS certification. Experience in home health, geriatrics, or hospice care is required.
