Key Responsibilities
The Hospice RN Case Manager is responsible for managing a primary caseload and providing skilled nursing care to patients and their families. This role ensures compliance with Medicare conditions of participation, state licensure laws, and agency policies while developing personalized care plans.
Requirements
Candidates must hold a valid Ohio Registered Nurse (RN) license and have 2 to 4 years of direct patient care experience in hospice or home care. A Bachelor’s degree in Nursing is preferred, though graduates of approved nursing programs are eligible.
