Physical Therapist - Full Time - Rehabilitation Services

Summary of Position

A Physical Therapist provides physical therapy services to patients utilizing
established policies and procedures, ensuring safe and effective care.  Services
are provided based on the ages of the population served; Children through
geriatric - 65 years or older and in collaboration with the multidisciplinary
care team.  Assesses, plans, implements and evaluates care in collaboration with
the multidisciplinary team.  Demonstrates the knowledge and skill necessary to
identify and meet the individualized needs of all patients served.  The Physical
therapist is under the supervision of the Director of Rehabilitation Services
and Rehabilitation Manager.

Qualifications

Education

● Graduation from an accredited Physical Therapy program with a baccalaureate,
masters or doctorate degree in Physical Therapy and successful passage of the
national licensure exam. 

Experience

● Two (2) years experience as a Physical Therapist preferred 

● Recent / previous work experience in out-patient rehabilitation setting
preferred

department needs are met 

● Must be flexible, dependable, and demonstrate the ability to adapt to change

Certifications/Licensure

● Current California or Nevada PT licenses and ability to obtain other state (CA
or NV) license within 3 months of date of hire. 

● Physical Therapist License Applicants may be considered, but full license for
State of California must be obtained within 3 months of employment and State of
Nevada license must be obtained within 6 months of employment.

● Current CPR/BLS certification by the American Heart Association (BLS for the
Healthcare Provider) or ability to obtain prior to start date.

Essential Functions

1. Provides consistently exceptional care at all times.

2. Performs initial and ongoing physical therapy evaluations according to APTA
standards of practice as well as state standards.

3. Develops a Plan of Care for each patient based on initial and ongoing
evaluations including short and long term goals, collaboration demands of other
staff members, and required patient and caregiver involvement.

4. Implements Plan of Care on a consistent schedule and adjust this schedule and
plan according to patient progress.

5. Documents patient progress according to approved medical documentation and
facility documentation procedures.

6. Evaluates mobility, strength and current level of patient function, while
setting goals to improve in these areas that are scaled to patient abilities and
realistically attainable.

7. Reports patient progress to patient, appropriate caregivers, facility staff
and healthcare professionals involved in ongoing treatment/care of patients.

8. Develops a plan for patients following completion of their Plan of Care with
appropriate referrals.

9. Assists other facility staff members as needed to provide care and create a
clean, orderly and effective environment for rehabilitation.

10. Assures continuation of therapeutic plan following discharge by designing
home exercise programs; instructing patients, families, and caregivers in home
exercise programs; recommending and/or providing assistive equipment.

11. Documents patient care services by charting in patient and department
records.

12. Maintains patient confidence and protects hospital operations by keeping
information confidential.

13. Maintains a safe and clean working environment by complying with procedures,
rules, and regulations.

14. Protects patients and employees by adhering to infection-control policies
and protocols.

15. Ensures operation of equipment by completing preventive maintenance
requirements, following manufacturer's instructions.

16. Maintains professional and technical knowledge by attending educational
workshops; reviewing professional publications; establishing personal networks;
participating in professional societies.

17. Develops physical therapy staff by providing information; developing and
conducting in-service training programs.

18. Complies with federal, state, and local legal and professional requirements
by studying existing and new legislation; anticipating future legislation;
enforcing adherence to requirements; advising management on needed actions.

19. Contributes to team effort by accomplishing related results as needed.

20. Occasional travel or rotation assignment to various health system locations.

21. Responds to the needs of the department by performing other duties, as
necessary.
