Provide high-quality, patient-centric, and cost-effective inpatient-level care within a home setting. Collaborate with a multidisciplinary team in a virtual Command Center to manage patient health and clinical outcomes.
Must be Board Certified or Board Eligible in Internal Medicine, Family Medicine, or Geriatrics. Previous experience as a hospitalist or in a skilled nursing facility is preferred.
