Key Responsibilities
Deliver skilled physical therapy to patients in their homes by developing and adjusting individualized treatment plans to improve mobility and function. Collaborate with an interdisciplinary team to coordinate care and educate patients and caregivers on safety and rehabilitation strategies.
Requirements
Requires a Master’s degree in Physical Therapy and a current Michigan PT license. Candidates must possess strong communication skills, a valid driver's license, and the ability to work independently in home environments.
