Job Description
Primary Care in the MCHS SEMN region is seeking a Nurse Practitioner or
Physician Assistant to work as part of a multidisciplinary team to provide care
to those living within our community through the Senior Service Skilled Nursing
Facility. Within in this position the NP/PA will work collaboratively with
primary care physicians to provide holistic care including acute, chronic,
preventive and palliative care services to patients in nursing homes and skilled
nursing facilities. The provision of care includes but is not limited to
comprehensive evaluation, ordering and interpreting diagnostic tests,
prescription of pharmacologic agents and treatments and non-pharmacologic
therapies, and counseling patients. Ideal candidates have experience in
Palliative Care, Primary care and/or Geriatrics.

Key Responsibilities
Provide holistic primary, acute, chronic, preventive, and palliative care to patients within skilled nursing facilities. Collaborate with primary care physicians to perform comprehensive evaluations and manage diagnostic tests and treatments.
Requirements
Requires a Master's degree for NPs or graduation from an accredited PA program, with at least 2 years of APP experience. Must hold valid MN state licensure, certification (FNP, ANP, GNP, or NCCPA), and active BLS and DEA registrations.
