Physical Therapist (PT) Details: Location: Dayton, OH & Surround Areas Schedule: PRN | Monday - Friday; Flexible Scheduling Pay per Visit: $60.00 - $120.00 (Based on Visit Type) Work Setting: On the Road for Patient Visits & Training at the Local Office or Remotely Territory: ~30-Mile Radius (Mileage Reimbursed) What You'll Do: As a PT, you’ll play a vital role in helping patients regain mobility and independence Delivering physical therapy services per physician’s care plan and state regulations Assessing patient mobility, strength, and rehabilitation potential Designing and implementing personalized therapy programs Educating patients and caregivers on therapeutic techniques and equipment Collaborating with interdisciplinary teams to ensure holistic care Supervising PTAs and home health aides as needed Documenting progress and participating in discharge planning

Qualifications

What We're Looking For Required: Active Ohio PT license Degree in Physical Therapy (Bachelor’s, Master’s, or Doctorate) CPR certification Valid driver’s license, reliable transportation, and auto insurance Recent TB test Preferred: 2+ years of PT experience Home health or community-based care experience Strong communication and organizational skills Ability to build rapport and lead with empathy Why Summit?
