Key Responsibilities
The Social Worker will act as a primary referral source, interviewing and coordinating resources to meet the social and emotional needs of residents and their families. They are responsible for maintaining resident rights, completing psychosocial evaluations, and facilitating care planning and discharge processes.
Requirements
Candidates must hold a Bachelor's degree in Social Work or a related human services field and possess a valid Texas Social Worker license or obtain one within six months. Two years of experience in long-term care or medical facilities is preferred.
