About the Role

Join SR COMPANIES LLC as a Home Health Speech Therapist – Orange Park in United States.

Key Responsibilities

  • Provide skilled speech pathology service, on an intermittent basis, to patients, in their homes, in accordance with the physician's orders.
  • RESPONSIBILITIES Evaluate the patient's level of functioning and recommend mechanisms to enhance the patient's hearing ability.
  • Provide initial, and ongoing, comprehensive assessments of the patient’s needs, including Outcome and Assessment Information Set (OASIS) assessments.
  • Establish a Speech Therapy treatment plan, in consultation with the physician, and revise it, with the approval of the physician, as necessary.

Requirements

  • language pathology, and who meets either of the following: Is licensed, as a speech
  • language pathologist, by the state in which the individual furnishes such services; or
  • In the case of an individual who furnishes services in a state, which does not license speech
  • language pathologist: Preferably a member of the American Speech
