The Nurse Practitioner provides high-quality primary care, diagnosing and managing acute and chronic illnesses within a person-centered model. They collaborate with interdisciplinary teams and PCPs to develop care plans that reduce hospital admissions and improve resident satisfaction.
Candidates must hold a Master of Science in Nursing, DNP, or NP certificate, along with active state and DEA licenses and national certification. A minimum of 2 years of clinical experience in geriatric, adult, or family practice settings is required.
