Key Responsibilities

• This includes counseling, motivational interviewing, and assessment for adherence packaging solutions.

• Provides and documents medication therapy management services.

• Establishes and maintains good working relationships with physicians and other health care professionals.

Requirements

• Provides prospective and retrospective DUR to ensure patient safety.

• Answers questions and provides information to pharmacy customers on drug interactions, side effects, dosage, and storage of pharmaceuticals.

• Reviews prescriptions issued by a physician or other authorized prescriber, to assure accuracy and assess the appropriateness of therapy.
