• Competitive pay starting at $33.55/hour+ — with higher rates for experience & licensure

In this role, you will:

• Support patients facing complex social, behavioral, and economic challenges
• Help remove barriers to care through resource connection and advocacy
• Work across programs including primary care, prenatal, substance use, and behavioral health
• Be part of a collaborative, team-based model focused on whole-person care

What You’ll Do

As a Social Worker – Float, you will:

• Connect patients to housing, benefits, and community resources
• Support patients across multiple programs and service lines
• Provide compassionate, confidential support in sensitive situations
• Communicate effectively across in-person, phone, and electronic platforms
• Document all patient interactions clearly and in a timely manner within the EHR
• Collaborate with providers and interdisciplinary teams to support patient care

What You’ll Bring

Required

• Bachelor’s Degree in Social Work or a related human services field
• Strong ability to maintain confidentiality and professionalism
• Excellent communication and interpersonal skills
• Ability to manage multiple priorities in a fast-paced environment

Preferred

• Experience working with vulnerable or underserved populations
• Experience in healthcare, behavioral health, or community-based services
