Lead the Plan of Care

• As a Case Manager, you're not simply responding to patient needs.
• You're anticipating them.
• You'll oversee each patient's episode of care, coordinate services across multiple disciplines, monitor progress, educate caregivers, and identify potential concerns before they become larger problems.
• Your clinical expertise becomes the foundation for safer recoveries and better patient outcomes.

What You'll Be Doing

• Manage a home health patient caseload throughout the St. Paul service area
• Complete OASIS assessments and develop individualized care plans
• Perform skilled nursing visits and comprehensive clinical evaluations
• Coordinate care with physicians, therapists, social workers, and other healthcare professionals
• Provide medication management, disease education, and ongoing patient support
• Monitor patient progress and adjust care plans as needed
• Complete documentation using Homecare Homebase (HCHB)
• Help patients remain safely at home while reducing avoidable hospitalizations
• Who We're Looking For
• Active Minnesota Registered Nurse (RN) license
• Three or more years of RN experience
• Home health experience preferred
• Excellent clinical assessment and critical thinking skills
• Strong communication and leadership abilities

Comfortable Managing an Independent Schedule

• Reliable transportation for travel throughout the St. Paul territory
