Key Responsibilities

• Maintains appropriate documentation.

• Generates initial treatment plans and updates every three (3) months. Discharge summaries are generated thirty (30) days after patient transferred, discharged or patient terminated treatment.

• Responsible for department operation during Director's absence.

• Representation of the Social Services Department and / or OHC as a liaison at agency and community meetings. May delegate some of this responsibility to other staff.
