Responsibilities
- Proactive, collaborative team member in a hospice care environment
- Conducts comprehensive adult/geriatric assessments of hospice patients and evaluates and manages plan of care
- Commitment to caring for patients and partners
- Complete all documentation and records in a timely and accurate manner
- Follow established policies and procedures as related to the scope of practice
- Maintain continuity of patient care in a hospice setting
Licensure, Certification, and Educational Requirements
Current, active, and unrestricted Registered Nurse (RN) and Nurse Practitioner (NP) licensure in state of practice
Board-certified by AANP or ANCC in adult, family practice, geriatric, or adult-gerontology primary care

Requirements
Must hold current, unrestricted RN and NP licensure in the state of practice. Board certification by AANP or ANCC in adult, family practice, geriatric, or adult-gerontology primary care is required.
