We are currently hiring a Physical Therapist (PT) to provide skilled therapy services to patients in their homes throughout Leesburg and surrounding areas.

Responsibilities

* Perform physical therapy evaluations and develop individualized treatment plans

* Provide skilled therapy, including therapeutic exercises, gait training, balance training, and mobility interventions

* Educate patients and caregivers on exercises, safety, fall prevention, and assistive devices

* Monitor patient progress and adjust treatment plans as needed

* Complete timely and accurate clinical documentation

* Communicate with physicians, nurses, and the interdisciplinary care team

* Supervise PTAs as required

Qualifications

* Graduate of an accredited Physical Therapy program

* Current Florida PT license in good standing

* Minimum 1 year of PT experience preferred

* Current CPR certification

* Valid driver's license, reliable transportation, and auto insurance

* Ability to meet all required background and compliance requirements
