Description

The Iowa Primary Care Association is actively recruiting a Family Medicine NP or PA provide care at a vibrant community health center (FQHC) in Waterloo, Iowa! This role performs primary care to the community providing a mix of well visits, acute care, & chronic disease management throughout the age spectrum.

The largest source of comprehensive primary care for underserved communities and populations.

With an integrated care model that includes medical, dental, behavioral health, and pharmacy services, patients can access comprehensive, coordinated services often under one roof.

Role Details

  • Clinic Hours: Mon, Tues, Fri 8am-5pm & Wed, Thur 8am-8pm
  • Schedule: 5 day work week with 1 evening per week (11:30a-8p)
  • Sees on average 12-14 patients per day

Provider team includes: 5 physicians (including the Co-Medical Director), 6 Nurse Practitioners, 2 Physician Assistant. This location also includes an OB clinic with 1 OB-GYN and 1 Certified Nurse Midwife

Requirements

  • Nurse Practitioner:
  • Graduation from an accredited/approved NP program
  • Licensed by the Iowa Board of Nursing as a NP

Physician Assistant

Graduation from an accredited AMA-approved PA program and/or certified by the National Commission on the Certification of Physician Assistants.

Licensed by the Iowa Board of Professional Licensure; licensed to practice in Iowa.

Key Responsibilities

The provider will perform primary care services including well visits, acute care, and chronic disease management for patients across the age spectrum. They will work within an integrated care model alongside a team of physicians, nurse practitioners, and physician assistants.
