Key Responsibilities

The Patient Navigator guides patients and families through the diagnosis, treatment, and follow-up phases of their care episode to ensure seamless transitions. They collaborate with multidisciplinary teams to manage education, psychosocial needs, and resources to achieve optimal clinical outcomes.

Requirements

Candidates must have completed an approved nursing education program and hold a valid Registered Nurse (RN) license in South Dakota. Basic Life Support (BLS) certification is required, and at least one year of experience in case management or patient care is preferred.
