The Nurse Practitioner manages a defined patient panel, performing clinical assessments and formulating longitudinal care plans for patients with chronic conditions. They collaborate with interdisciplinary teams and external providers to ensure care alignment and improve patient outcomes across home, clinic, and telehealth settings.
Candidates must hold a Master’s degree in Nursing and possess at least 2 years of experience as a Family or Primary Care Nurse Practitioner with a focus on cardiology or nephrology. A current state license, national board certification, and a valid driver's license for regional travel are required.
