The Nurse Practitioner manages a defined patient panel to provide longitudinal care for chronic conditions like CHF and CKD/ESKD. They collaborate with interdisciplinary teams to formulate treatment plans, perform physical exams, and ensure care plan alignment across various settings.
Candidates must hold a Master’s degree in Nursing and possess at least 2 years of experience as a Nurse Practitioner in primary care, cardiology, or nephrology. A current state license, national board certification, and a valid driver's license for regional travel are required.
