Key Responsibilities
The physical therapist will initiate evaluations and develop care plans to provide one-on-one rehabilitation services in patients' homes. They will also document clinical progress, educate caregivers, and participate in interdisciplinary discharge planning.
Requirements
Candidates must have at least one year of experience and a current, unrestricted California physical therapy license. A current CPR/BLS certification and reliable transportation are also required.
