As a Home Health Physical Therapist, you are a critical part of the care team - helping patients safely recover, regain mobility, and maintain independence where they feel most comfortable: at home.

  • What You’ll Do
  • Deliver Patient-Centered Care
  • Evaluate and assess patients’ functional status, mobility, strength, and rehabilitation potential
  • Develop and implement individualized care plans in collaboration with physicians and the care team
  • Provide hands-on therapy to improve mobility, strength, balance, and independence
  • Educate patients and caregivers on treatment plans, exercises, and safety strategies
  • Drive Outcomes & Recovery
  • Monitor patient progress and adjust treatment plans as needed
  • Support safe transitions of care and contribute to discharge planning
  • Identify additional patient needs and collaborate with interdisciplinary teams
  • Collaborate & Communicate
  • Partner closely with nurses, therapists, and physicians to ensure coordinated care
  • Document patient care and progress accurately and timely within the EMR
  • Participate in case conferences and team meetings
  • Lead & Support
  • Supervise Physical Therapist Assistants (PTAs) in accordance with state guidelines
  • Provide guidance and education to caregivers and support staff

What You Bring

  • Graduate of an accredited Physical Therapy program (DPT or Master’s)
  • Current PT license in the state of practice
  • Minimum 1–2 years of experience (home health experience preferred)
  • Strong clinical assessment and critical thinking skills
  • Excellent communication and patient education abilities
  • Current CPR certification
  • Valid driver’s license, reliable transportation, and auto insurance

Key Responsibilities

The Physical Therapist will evaluate patient functional status and develop individualized care plans to improve mobility, strength, and independence in a home health setting. They will also collaborate with interdisciplinary teams, document progress in the EMR, and supervise Physical Therapist Assistants.

Requirements

Candidates must hold a degree from an accredited Physical Therapy program and possess a current state PT license. A minimum of 1–2 years of experience, preferably in home health, along with current CPR certification and a valid driver's license are required.
