Responsibilities
- vision of a physician, the nurse practitioner visits patients, completes physical assessments, performs face-to-face assessments and evaluates hospice eligibility as assigned in consultation with the hospice interdisciplinary team. The nurse practitioner builds from the resources of the community to plan and direct services to meet the need of individuals and families within their homes and communities. Accreditation from an approved certifying body for advanced practice nursing as required by State. In good standing with the OIG Will need to be available to meet the regulatory requirements of the face-to-face schedule. This includes urgent requests for admissions. Serve as an educational resource to the hospice staff, patients and families as needed
Key Responsibilities
The Nurse Practitioner provides hospice care by evaluating patient health history, diagnosing medical conditions, and developing comprehensive care plans. They perform face-to-face assessments and collaborate with an interdisciplinary team to meet the needs of patients in their homes.
Requirements
Candidates must have graduated from an accredited School of Nursing and an accredited Nurse Practitioner Program. They must also hold valid certification for advanced practice nursing and be in good standing with the OIG.
