The RN Diabetes Educator and Case Manager is a Registered Nurse who is trained and preferably certified as a diabetes educator.

This Registered Nurse will provide comprehensive diabetes self-management education and support to NVHC patients.

This role requires the ability to work collaboratively with providers, clinical pharmacists, case managers, and other team members to promote and support a team-based care model. As a diabetes educator, this RN will assess patient needs, develop individualized care plans and participate in interdisciplinary care meetings. As a case manager this RN works to improve access and effectiveness of care delivery system to improve patient outcomes.

Ability to travel within the State of Nevada.

Essential Duties and Responsibilities

• Direct patient education in-person and virtually.
• Provide medication and insulin teaching to patients.
• Provide glucose monitoring support to patients.
• Provide lifestyle counseling to promote optimal self-management with patients with Diabetes and other chronic diseases.
• Provide and participate in care coordination with primary care and specialists
• Coordinate and conduct home visits as needed.
• Interpret data and trends from multiple data sources including remote patient monitoring, using appropriate analytical and critical thinking skills.
• Provide timely and accurate documentation, tracking and reporting of diabetic clinical outcomes.
• Provides timely responses to inquiries from health plans, providers and other members of the patient care team.
• This RN will collaborate with medical, dental and operational leadership to develop a comprehensive Medical assistant training curriculum.
• Other duties as assigned by directed.

Required Qualifications

• Graduate of an accredited nursing school
• Licensure as a registered nurse in the state of Nevada
• Current CPR or BLS certification
• Active Nevada Driver's License
• A minimum of three years of experience in a clinical practice with good clinical skills/ability to function as a staff nurse is required

Preferred Qualifications

• Knowledge of the essential functions, practices and procedures of a medical clinic and office
• Knowledge and skill in pediatric and adult medication and immunization administration and storage and handling practices.
• Excellent ability to problem solve, de-escalate/resolve conflict and perform service recovery.
