- Licenses: Colorado Medical Board, Drug Enforcement Administration
- Certifications: BLS, ACLS, PALS, ATLS
- Experience: Credentialed by FHW Medical Staff

Essential Functions
GENERAL DUTIES:
- Reliable and punctual attendance is an essential job function for every FHW employee.
Employees are expected to be at their job as scheduled.
- Complete all required hospital and insurance credentialing promptly upon request.
- Be responsible for complying with the applicable legal requirements for a Physician to obtain and/or maintain prescriptive authority in Colorado.
- Comply with policies, laws, and accreditation standards.
- Complete all required FHW training within 90 days of employment and annually thereafter. The required training consists of, but is not limited to, Safety, HIPAA, Resident Abuse, CPR, MSDS, and New Employee Orientation (attendance only required once for NEO).
- Perform the normal and customary functions of a physician practicing Family Medicine.
- Collaborate with staff, physicians, and administrative staff to determine the most appropriate site of care for patients and most cost-effective use of resources.
- Provide prompt and clear communication about patient admission, condition changes, and discharge to clinicians responsible for outpatient care.
- Complete all patient charts and documentation, written and phone orders, and other information necessary for billing laboratory, imaging, and other ancillary services within reasonable specified time frames.
- Provide the full-range of services regardless of patient’s ability to pay.
- Agree to prescribe all appropriate medications needed by patients including narcotics.
- Provide physician input into improvement of quality, patient safety, and patient and family satisfaction.
- Actively participate in practice and organizational level quality programs as appropriate, and participate in all meetings and conferences designated by Employer as “mandatory,” unless excused by Employer.
- Provide additional duties to include but not limited to marketing, training, and consulting for Employer.
- Act with a high degree of corporate responsibility relating to the use of resources, equipment and supplies to insure high quality patient care and fiscal responsibility.
- Promote cooperation and teamwork among other physicians, support staff, and FHW personnel.
- Direct other employees of the Employer in the provision of services, and adhere to all reasonable job descriptions, policies, procedures, and performance standards adopted by the Employer.
- Communicate necessary information to others as appropriate.
- Maintain patient/resident/employee privacy and confidentiality at all times.
- Actively support Service Excellence in all interactions with patients, staff, and administration.
- Represent Physician Services through positive, professional communication and interactions with staff, patients, family members, the community, and other FHW departments.
- Demonstrate leadership to align with FHW mission, vision, and values in all daily roles and responsibilities.
- In attrition to the above duties, Physician will comply with Employment Agreement and subsequent Amendments in their entirety.

Guaranteed Base Salary $237,000

Key Responsibilities
The physician will perform customary Family Medicine duties, including patient examinations, documentation, and collaborative care planning. They are also responsible for participating in quality programs, maintaining hospital credentialing, and providing leadership to support the organization's mission.
Requirements
Candidates must hold a Board Certification in Family Medicine and possess active Colorado medical and DEA licenses. Additionally, they are required to maintain certifications in BLS, ACLS, PALS, and ATLS.
