Key Responsibilities
The RN Case Manager will provide compassionate hospice care to patients and partner with them to improve their health outcomes. The role involves managing patient cases and operating a vehicle to visit patients as part of the care delivery process.
Requirements
Candidates must have at least one year of clinical nursing experience and hold a current RN license in Oklahoma or a compact state. An associate or bachelor's degree in nursing is required, along with a valid driver's license and BLS certification.
