Key Responsibilities
- Provide assessments and treatment plans for patients in the Primary Care Offices with targeted treatments or planning interventions with patients.
- Provides care coordination for patient’s primary care provider/specialist as requested
- Interventions meet patient/family needs as identified by psychosocial assessment.
- All interventions will be documented in medical record.
