Key Responsibilities

• Provide skilled physical therapy services to patients in their homes as ordered by the attending physician.

• Counsel patient and family on the disease/injury and how to manage in all phases of physical therapy.

• Oversee and supervise therapy services of patient provided by PTAs and when appropriate, nurse assistants.

Requirements

• Required PT Home Health experience: 1 year within the last 3 years

• Required Home Care Home Base (HCHB) charting experience – Required OASIS experience

• Preferred Must be Covid Vaccinated or provide medical or religious exemption Education, License and Certification Requirements: Bachelor of Science in Physical Therapy or Similar

• Required Current State License as Physical Therapist or Compact if applicable

Work Schedule

• Contract position.
