Key Responsibilities
- provide services to meet the needs of individuals and families within their homes.
- Develop an individualized care plan, which establishes measurable goals based on the therapy assessment incl.
- Provide physical therapy instructions and HEP education to patient/family/caregivers as appropriate.
- implement prior to discharge of patient.
Requirements
- A degree from a baccalaureate or master’s program in physical therapy approved by an accredited organization.
- Licensed to practice as physical therapist within the state.
- Two years of appropriate experience as a physical therapist.
- Community/home health experience is preferred.

Work Schedule
- Part-Time position.
- No weekends required.
