About the Role
The Roads Home Health Care is hiring a Home Health Occupational Therapist to help patients throughout South Florida regain independence and function in their own homes. As an occupational therapist on our team, you'll evaluate patients, design individualized occupational therapy treatment plans, and provide hands-on care and supervision for occupational therapy assistants. Both full-time and per diem opportunities are available for candidates who want flexibility alongside meaningful, one-on-one patient care.
* Perform comprehensive in-home occupational therapy assessments and reassessments
* Obtain and communicate physician orders for certification and recertification of occupational therapy services
* Deliver occupational therapy treatment appropriate to each patient's clinical status, needs, and age
* Design home exercise and activities-of-daily-living programs tailored to each patient's goals
* Evaluate and manage patient pain using appropriate occupational therapy interventions
* Coordinate care planning with physicians, patients, families, and the broader care team
* Educate patients and caregivers on treatment techniques and home safety strategies
* Provide clinical supervision to occupational therapy assistants
* Complete timely, accurate documentation and participate in interdisciplinary team conferences
* Assess the effectiveness of occupational therapy services and adjust treatment to support optimal patient outcomes
Job Requirements
* Current, valid Florida Occupational Therapist license
* Graduate of an ACOTE-accredited occupational therapy education program
* NBCOT certification (National Board for Certification in Occupational Therapy)
* Minimum of two years of occupational therapy work experience
* Current CPR/BCLS certification, current health certificate, and TB clearance
* Valid driver's license, reliable vehicle, current auto insurance, and ability to pass a criminal background check
Qualifications
* Strong clinical judgment and ability to design effective, individualized treatment plans
* Excellent communication skills for patient and family education and interdisciplinary coordination
* Ability to work independently while managing a home visit caseload
* Compassionate, patient-centered approach to occupational therapy and rehabilitative care
* Flexible and organized, with sound clinical decision-making skills
* Genuine commitment to helping patients regain independence in everyday activities
* Prior home health experience preferred
* Bilingual candidates a plus
