Provide psychosocial assessments and emotional support to hospice patients and their families throughout their end-of-life journey. Coordinate with the interdisciplinary team to develop and implement patient care plans while maintaining accurate medical documentation.
Requires a Master’s degree in Social Work and a current LICSW or LCSW license in Massachusetts. Candidates should have at least two years of healthcare or hospice experience and a valid driver's license for travel.
