Job Details: Job Location: Redwood Coast PACE Eureka - Eureka, CA, Position Type: Full Time, Salary Range: $222915.00 - $250890.00Salary, Job Shift: Day, Redwood Coast PACE with Humboldt Senior Resource Center is looking for either a part-time or full-time Primary Care Physician to provide care for our participants.

Redwood Coast PACE — a Program of All-inclusive Care for the Elderly — is a comprehensive health plan for older adults who wish to remain living in their home and community. We provide medical, social, nutritional, rehabilitation, and support services for eligible adults.

At Redwood Coast PACE, the goal is to help people stay as healthy and independent as possible. Because their health is monitored on a regular basis, many PACE participants report fewer health crises and better overall health.

JOB SUMMARY: Under the general supervision of the Chief Medical Officer, provides comprehensive primary care and medical management to Redwood Coast PACE participants in accordance with professional standards, applicable laws and regulations, and PACE program requirements.

Works collaboratively with the Interdisciplinary Team (IDT), Clinic team, Nurse Practitioners, Registered Nurses, specialists, hospitals, skilled nursing facilities, hospice, assisted living facilities, and other care providers to coordinate and ensure continuity of participant care.

Provides clinical care across a variety of settings, including the PACE clinic, participants’ homes, residential care settings, skilled nursing facilities, hospitals, and other appropriate care settings.

Participates in the development and oversight of individualized Plans of Care, supports quality improvement and program development activities, and provides medical consultation and leadership to the clinical team.

ESSENTIAL DUTIES & RESPONSIBILITIES: Performs initial assessment, including comprehensive history and physical, of assigned participants at intake, and at least every 6 months in cooperation with family member and/or caregiver and Interdisciplinary Team (IDT) members.

Develops and implements a therapeutic Plan of Care at least every 6 months and when participant conditions change. Monitors Plan of Care on an ongoing basis. Presents and coordinates the Plan of Care with the IDT.

Helps ensure that participants experience a smooth transition into Redwood Coast PACE by coordinating with previous medical providers. Provides evaluation, treatment, and ongoing monitoring of episodic illness. Refers participants to medical specialist as indicated.

Provides oversight of hospitalized participants (in collaboration with a hospitalist as appropriate) including, but not limited to providing primary care responsibilities for management, providing updates to the team on a regular basis, judiciously utilizing medical specialty consultants, and coordinating discharge planning with the IDT.

Provides primary care to participants in accordance with professional standards and applicable laws and regulations.

Provides care in a variety of settings, including the clinic, skilled nursing facilities, participants’ homes, assisted living facilities, hospice inpatient units, and hospitals, as appropriate to meet participant care needs.

Maintains timely and effective communication and coordination with hospital, skilled nursing, hospice, and assisted living staff to support continuity and quality of participant care. Collaborates with internal Nurse Practitioners by providing consultation and medical leadership.

Participates as an active member of the IDT by attending IDT meetings, family meetings and clinical case conferences, consulting with Nurses and other team members as medical care issues arise, and attending team and other meetings, as applicable.

Provides on-call medical coverage to participants after normal Clinic business hours. Participates actively in Redwood Coast PACE Quality Assessment and Performance Improvement (QAPI) activities. Works closely with Redwood Coast PACE Nurse Practitioners and Registered Nurses to provide clinical care.

Assigns, orders, and delegates appropriate nursing interventions to Registered Nurses, as determined by the participant’s Plan of Care. Participates in primary care program development and implementation as determined by the needs of Redwood Coast PACE.

Understands fiscal issues and is appropriately involved in meeting budget targets. Complies with safety policies and procedures, including identifying and immediately reporting unsafe acts or conditions to the Chief Medical Officer and Director of Redwood Coast PACE.

Takes necessary measures to ensure a safe environment. Continually seeks better ways for delivering services and communicating with participants. Understands the importance of community involvement and participates as appropriate in activities that link Redwood Coast PACE to the community.

Effectively collaborates with staff peers and contractors to meet Redwood Coast PACE goals and further success. Complies with all policy and procedures of Redwood Coast PACE. Participates in the process of teaching, training, and mentoring student interns, and volunteers working within the program area.

Acts in a manner consistent with agency values. Additional duties may be assigned, as needed, that align with the responsibilities of the position and support the vision, mission, and values of the organization.

Qualifications

EDUCATION &/or EXPERIENCE: Graduate of an accredited School of Medicine. Board certified in Internal Medicine or Family Practice. Board certification granted by the American Boards of Internal Medicine and Family Practice. Certificate of Added Qualifications in Geriatric Medicine (CAQ) preferred.

Two (2) years of experience working with a frail or elderly population required, preferably in a hospital or long-term care facility. Experience working as part of an IDT preferred. LICENSES OR CERTIFICATES: Licensed by the California Medicine Board to practice medicine in California.

Board certification in internal medicine or family practice plus current Drug Enforcement Administration (DEA) Registration Number required. Valid California Driver’s License and personal auto liability insurance preferred.

Personal vehicle in good driving/operating condition, insured for State minimum liability requirements that can be used for the applicable job functions noted above. PRE-SCREENING REQUIREMENTS: Pre-employment physical, and PPD skin test (TB screening). COVID-19 and Influenza (flu) vaccinations preferred.

This position requires the successful completion of a background check. A current Motor Vehicle Report (MVR) is required to determine eligibility for driving on behalf of the organization.

JOB SKILLS: Medical knowledge and clinical skills necessary to provide comprehensive, interdisciplinary care to frail, elderly, and disabled adults, including the ability to manage complex clinical conditions while promoting participant dignity, independence, and quality of life.

Knowledge and experience working with physical, social, and mental health programs operating within a licensed health facility or clinic. Demonstrates respect for and promotes participants’ rights including dignity, self-determination, access to care, confidentiality, and independence.

Ability to function effectively within a multi-service, community-based nonprofit agency. Ability to comply with program requirements and applicable government regulations.

Strong verbal and written communication abilities; able to convey information clearly and professionally to staff, participants, families, and external partners. Builds positive relationships with diverse populations, including aging and dependent adults, families, caregivers, staff, and community stakeholders.

Works effectively as part of a multidisciplinary team demonstrating sound listening skills to receive and give directions and understand participant concerns; maintains a cooperative and supportive attitude. Applies good judgement, resourcefulness, and flexibility to resolve issues and adapt to changing circumstances.

Maintains confidentiality, reliability, and ethical standards; consistently demonstrates punctuality, attendance, and responsibility in work performance. Maintains participant privacy and organizational confidentiality, adhering to HIPAA and agency standards.

Remains calm and effective when priorities shift, deadlines change, and unexpected issues arise. Ability to work independently, organize and prioritize tasks, manage time effectively, and meet deadlines with minimal supervision while maintaining accuracy and accountability.

Ability to be dependable, accept supervision, follow direction, and improve performance through feedback. Ability to incorporate an understanding of detailed requirements in work activities. Ability to develop and implement action plans. Ability to collect and analyze data.

Ability to coordinate, advocate, and cooperate with other agencies for seniors with tact and diplomacy. Ability to speak to public and private community groups about agency programs and to participate in various outreach and fundraising efforts as required.

Strong basic computer skills (Microsoft Outlook, Word, Power Point, and Excel). Ability to navigate, use, and maintain database software programs. Basic knowledge of web-based applications such as timekeeping, training, and email programs.

PHYSICAL REQUIREMENTS: The position requires the ability to sit, stand, walk, bend, stoop, reach, and move throughout clinic and community-based care settings for extended periods.

The employee must be able to travel between work locations and enter and navigate participants’ homes, hospitals, skilled nursing facilities, assisted living facilities, and other care settings.
