Job Description
Jefferson Health at Home by BAYADA Hospice is hiring a compassionate and dedicated per-diem Nurse Practitioner, (NP) to join our team. This office services our hospice clients throughout Bucks County, Montgomery County, and Northeast Philadelphia.
Prior home care, hospice, or palliative care experience strongly preferred, but not required. As a hospice nurse NP, you will be an integral member of a multi-disciplinary health care team that provides palliative care to clients, affording them the opportunity to receive the medical care required to remain at home.

Nurse Practitioner (NP) Responsibilities
- Making home face to face hospice recertification visits to clients in designated geographic territories.
- Monitoring clients' conditions; reporting changes to Clinical or Client Services Manager.
- Following up with, executing, and properly documenting doctors' orders.
- Performing client assessments as necessary.
Qualifications for a Nurse Practitioner (NP)
- A current license as a NP in PA
- A minimum of one year of recent, verifiable clinical (medical/surgical) experience.
- Prior home care, hospice and/or palliative care experience strongly preferred, but not required.
- Solid computer skills; prior experience with electronic medical records (EMR) preferred.
- Ability to travel to cases as assigned.
- Strong interpersonal skills.

Key Responsibilities
The Nurse Practitioner will conduct face-to-face hospice recertification visits and monitor client conditions within designated territories. They are responsible for reporting changes to management and properly documenting all medical orders and assessments.
Requirements
Candidates must hold a current Nurse Practitioner license in Pennsylvania and possess at least one year of recent clinical experience. Proficiency with electronic medical records and the ability to travel to assigned cases are required.
