Key Responsibilities

Provide in-home physical therapy visits, including evaluations, reassessments, and fall risk assessments. Maintain accurate documentation and communicate patient progress effectively with the care team.

Requirements

Candidates must hold a current Hawaii license as a Physical Therapist or Physical Therapist Assistant. A minimum of one year of clinical experience and a valid driver's license with reliable transportation are required.
