The Nurse Practitioner supports patients and families during the final stages of illness, offering thoughtful medical care and a steady, compassionate presence. They help families understand what to expect and guide them in making informed choices that align with the patient’s goals and wishes.

Qualifications
Nurse Practitioner Requirements & Key Responsibilities: Hold a current Nurse Practitioner (NP) or Advanced Practice Registered Nurse (APRN) license in the state(s) of practice. Degree from an accredited NP program required. Minimum two years of RN experience; preferred hospice or palliative care experience.
Provide comprehensive medical care for terminally ill patients, including assessments, diagnostic interpretation, and individualized care plans. Prescribe medications/treatments, complete hospice recertification face-to-face assessments, and ensure compliance with state and federal regulations.
Collaborate with interdisciplinary teams and physicians to manage symptoms and maintain continuity of care. Educate and support patients, families, and caregivers on end-of-life care, including medical, emotional, and spiritual needs.
Maintain accurate documentation, adhere to HIPAA standards, and participate in quality improvement initiatives. Demonstrate strong clinical judgment, communication, and problem-solving skills; proficient in electronic health records.
Valid driver’s license, reliable transportation, automobile insurance, and ability to pass background and drug screening required. The salary for this role is: $35 per hour for orientation, meetings and required education and $100 pe face to face visit.
An employee’s actual annual salary will be based on but not limited to: geographic location, relevant education, applicable certifications, depth of experience, and special skillsets.
The total compensation package for this position may also include the opportunity to participate in employee benefits including our medical/dental/vision/pet insurance, disability and life insurance, paid time off, and 401(k) retirement plans, subject to applicable plan terms, and/or eligibility for other compensation.
