Highlights

Community Physician Network is actively seeking a Board Certified/Board Eligible Family Medicine Physician to join our outpatient practice at the Stones Crossing Pavilion. The pavilion is part of a rapidly growing medical facility that will be expanding to meet the needs of the surrounding community, offering exciting future opportunities for our care teams.

Licensure & Certifications

  • Board Certified or Board Eligible in Family Medicine
  • Licensed in Indiana or eligible for medical licensure in Indiana

  • Ability to develop and maintain relationships with referring physicians

Key Responsibilities

The physician will provide family medicine services within an outpatient practice setting as part of a team-based care model. They will collaborate with a large group of specialists and utilize the EPIC EMR system to manage patient health.

Requirements

Candidates must be Board Certified or Board Eligible in Family Medicine and hold or be eligible for an Indiana medical license. Excellent communication skills and a commitment to patient-centered values are required.
