Key Responsibilities

• Assess clients' needs, strengths, and challenges through interviews and evaluations
• Develop and implement individualized care plans in collaboration with clients and other professionals
• Provide counseling and emotional support to clients facing various social, emotional, or behavioral issues
• Connect clients with community resources, services, and support networks
• Advocate for clients' rights and assist with navigating complex systems such as healthcare, housing, and social services
• Maintain accurate and confidential case records in accordance with organizational policies
• Monitor and evaluate client progress, adjusting care plans as necessary
• Collaborate with multidisciplinary teams to ensure comprehensive support for clients

Requirements

• Bachelor's degree in Social Work, Psychology, Sociology, or a related field; a Master’s degree is preferred
• Valid state licensure or certification as a Licensed Clinical Social Worker (LCSW) or equivalent (if applicable)
• Proven experience in social work, counseling, or related roles
• Strong interpersonal and communication skills
• Ability to handle sensitive situations with empathy and professionalism
• Knowledge of social service resources, community programs, and relevant laws and regulations
• Excellent organizational and record-keeping abilities
• Ability to work independently and as part of a team
