Key Responsibilities

Conduct in-home health evaluations for patients, including physical assessments and medication reviews. Provide connections to appropriate care and document findings using an intuitive clinical workflow on an iPad.

Requirements

Must hold an active, unrestricted medical license in the coverage area and be capable of obtaining multi-state licenses. Candidates must be comfortable traveling up to 100% of the time and performing duties in diverse home environments.
