Nurse Practitioner

Join Community LIFE as a Nurse Practitioner in Bedford, Pennsylvania. Community LIFE is changing the way our communities view aging. We are a Program of All-Inclusive Care for the Elderly, which integrates population health management and comprehensive health and social services (operating as LIFE – Living Independently for the Elderly).

Key Responsibilities

• Pennsylvania RN license

• CRNP certification in Adult-Gerontology Primary Care Nurse Practitioner (AGPCNP) or Family Nurse Practitioner (FNP),

• Pennsylvania Prescriptive Authority Approval

• DEA license.

Requirements

• Geriatric Assessment and management of co-morbidity.

• Effective listening and communication skills with patients, families and IDT members.

• Clear and complete documentation.

• Effective Team skills including but not limited to reliability, active listening, positive approaches, courtesy and respect in line with Community LIFE Mission and Values statements.

Benefits & Compensation

• Salary: $100K - $125K

• Full-Time position.
