Key Responsibilities

• Performs initial screening and evaluation of the psychosocial, emotional, and spiritual status of patients and families/caregivers related to the patient’s terminal illness and environment.

• Documents assessments, visits, telephone contacts, referral actions and social service interventions in the medical record clearly and concisely in a timely manner.

• Provides short term crisis intervention and individual or family counseling when indicated. Utilizes de
