Key Responsibilities

Conduct comprehensive psychosocial assessments

Participate in individualized treatment planning and care coordination

Provide clinical social work services and ongoing case management

Link clients to community resources including healthcare,

Requirements

Collaborate with internal and external providers to support client stability and recovery Qualifications Education and Licensure

Master’s degree in social work (MSW) required Experience and Skills

Experience working with individuals with serious mental illness strongly preferred

Experience in behavioral health, mental health, long term care, or human services settings

Salary: $29.72/hr

Work Schedule

Full-Time position.

Weekend coverage may be required.

Day shift only - no nights.
