Social Worker

Key Responsibilities

The Social Worker participates in the planning, implementation, and evaluation of care plans for older adults within a PACE model. They provide psychosocial assessments, supportive counseling, and facilitate transitions between home, hospital, and long-term care settings.

Requirements

Candidates must hold a current Massachusetts L.I.C.S.W. or L.C.S.W. license and have at least one year of experience in clinical or case management with a geriatric population. The role requires the ability to perform local travel and pass a respirator fit test.
