Conduct comprehensive in-home health assessments for Medicare Advantage members, including physical exams and medication reconciliation. Collaborate with primary care providers to address care gaps and educate patients on disease processes and health management.
Requires an active ANCC/AANP certification for NPs or NCCPA certification for PAs, along with unrestricted state licensure. Candidates must possess a valid driver's license and reliable transportation to perform field-based home visits.
