VNA Hospice Registered Nurse Case Manager - RN

• Registered Nurse Case Manager - RN | Legacy Lifecare VNA/Hospice
• $41.00 - $43.00/hour, based on experience
• Full-Time | 40hrs per week
• Monday – Friday | 8:00AM – 5:00PM | Occasional On-Call Rotations

Delivering Solutions that Matter

• Provide kind, compassionate care to residents in home or assisted living settings.
• Observe residents and chart/report changes such as response to medication or treatment.
• Perform basic resident care or treatments, including temperature and blood pressure monitoring

Support palliative and hospice care needs, including pain management, symptom control, and emotional/spiritual support

Qualifications for Success

• Current, unencumbered MA RN License
• Minimum 1-year RN experience
• Prior experience in home-care, hospice, rehabilitation, or long-term care setting preferred
• Reliable transportation and the ability to travel to patient homes or hospice facilities.
• Ability to communicate fluently in English – both verbally and written
