Key Responsibilities

• Provide Social Work assessment, intervention and evaluation at an advanced practice level for complex and general caseload/s.

Requirements

• Contribute to effective and efficient consumer discharge planning via timely collaboration and communication with the multi-disciplinary team, the consumer and their careers.

• Successful applicants will have tertiary qualifications in Social Work and be eligible for membership with the Australian Association of Social Work and eligible for Mental Health credentialing.
