Key Responsibilities

Provide one-on-one physical therapy to older adults in their homes or senior living communities while developing individualized plans of care. Collaborate with an interdisciplinary team and utilize telehealth tools to support patient mobility, independence, and wellness.

Requirements

Candidates must hold a valid Physical Therapy license or be eligible to obtain one and possess a degree from an accredited Physical Therapy program. Proficiency in basic computer skills, EMR systems, and current CPR certification are required.
