Key Responsibilities
The Nurse Practitioner will consult with patients and families regarding disease processes and end-of-life care while collaborating with the interdisciplinary team to develop care plans. Requirements
Candidates must hold a current state license as a Registered Nurse and Advanced Practice Nurse, along with a Baccalaureate degree in Nursing. A minimum of two years of clinical experience in an APN role, preferably in hospice or home health, is required.
